Application for Membership to Join the BFVC
Applications for membership in the Beth-El Fellowship of Visionary Churches may be obtained in the following ways:
Part I: Application Form (multiple pages)
In Person (Pick up an Application Form for Membership at The House of the Lord® Church Office)
By Email (We can email the Application Form to you. Send your email request to us by sending a message using the Contact Us page on our website, or by contacting Pastor Dennis Butts, Executive Director of Beth-El Fellowship of Visionary Churches at dennis@thotl.org.
In Writing (Write to the address listed on the Contact Us page of this website.)

Part II: Ecclesiastical Member Affiliation Agreement
After prayerfully reviewing your Application, the Ecclesiastical Member Affiliation Agreement (in booklet form) will be sent to you to review. In many cases, the Agreement is reviewed and discussed with the Executive Director or President and CEO of Beth-El Fellowship of Visionary Churches for clarification and to answer any questions.
Part III:
Signature
Both parties, i.e. an Authorized Representative (Executive Director or President and CEO) of Beth-El Fellowship of Visionary Churches and the Senior Pastor (Church or parachurch ministry founder/pastor) sign and date the Covenant Agreement.
